| When to suspect stenosis, Adult-Derived · ASE 2024, Table 5 Significant stenosis should meet at least one flow-dependent (velocity, mean gradient) and one flow-independent (EOA or DVI) parameter. The guideline's Figure 13 algorithm uses other cut-offs (DVI ≥0.30 normal, 0.29–0.25 possible, <0.25 stenosis; acceleration time split at 100 ms); this table follows Table 5. How to measure and caveats
The guideline presents its approach as one developed in adults and says it generally works in congenital heart disease, with limits: serial stenoses, shunts, eccentric jets, noncircular outflow tracts and body size (Section VI, Table 23). | |||
|---|---|---|---|
| Measure | Normal | Possible stenosis | Suggests significant stenosis |
| All prosthetic aortic valves | |||

| Mean gradient (mmHg) | <20 | 20–34 | ≥35 |
| DVI | >0.35 | 0.25–0.35 | <0.25 |
| EOA | Reference EOA ± 1 SD | 1 SD smaller than reference EOA | 2 SDs smaller than reference EOA |
| TAVI: change from baseline | |||
| Mean gradient | Change <10 mmHg | Increase of 10–19 mmHg | Increase ≥20 mmHg |
| DVI | Change <0.1 or 20% | Decrease 0.1–0.19 or 20%–39% | Decrease ≥0.2 or ≥40% |
| EOA | Change <0.3 cm² or 25% | Decrease of 0.3–0.59 cm² or 25%–49% | Decrease ≥0.6 cm² or ≥50% |
Some normal values in Table A4 cross these cut-offs (mean gradient 25.3 ± 8.0 mmHg for the 19-mm ATS). Judge against that valve's own normal values and the baseline study, as the guideline advises.
| Prosthesis–Patient Mismatch (PPM), Adult-Derived · ASE 2024, Table 7 PPM: a normally functioning prosthesis too small for the patient. Valve structure and motion are normal and the measured EOA is within 1 SD of the reference EOA; indexed EOA is the continuity EOA divided by BSA. Gradients rise steeply when indexed EOA falls below 0.8–0.9 cm²/m². In children with valve disease, repair is preferred because PPM is inevitable as they grow (Section VI). The guideline gives no pediatric-specific PPM cut-offs. | |||
|---|---|---|---|
| Indexed EOA (cm²/m²) | Normal | Moderate | Severe |
| BMI <30 kg/m² | >0.85 | 0.85–0.66 | ≤0.65 |
| BMI ≥30 kg/m² | >0.70 | 0.70–0.56 | ≤0.55 |
| Finding | Possible | Significant |
|---|---|---|
| Mean gradient | Increase ≥10 mmHg to a mean gradient ≥20 mmHg, with a decrease in EOA ≥0.3 cm² or ≥25% and/or in DVI ≥0.1 or ≥20% | Increase ≥20 mmHg to a mean gradient ≥30 mmHg, with a decrease in EOA ≥0.6 cm² or ≥50% and/or in DVI ≥0.2 or ≥40% |
| Intraprosthetic AR | New, or worse by at least one grade, reaching moderate or greater | New, or worse by at least two grades, reaching moderate or greater to severe |
With combined stenosis and regurgitation, significant deterioration may be present at lower thresholds. The guideline notes that its criteria differ slightly from other published criteria.

As printed: Medtronic Advantage and Medtronic-Hall each appear twice, and the table lists both Edwards Mosaic and Medtronic Mosaic.
Our note: several homograft cells look out of place (a peak gradient of 1.7 ± 0.3 mmHg beside an EOA of 5.8 cm²); check the original study before relying on them.
| When to suspect stenosis, Adult-Derived · ASE 2024, Table 11 High velocity, VTI ratio and mean gradient with a normal PHT point to significant prosthetic MR rather than stenosis (mechanical valves, Table 12).
| Peak velocity (m/s) | <1.9 | 1.9–2.5 | ≥2.5 |
| Mean gradient (mmHg) | ≤5 | 6–10 | >10 |
| VTIPrMV / VTILVOT | <2.2 | 2.2–2.5 | >2.5 |
| EOA (cm²) | ≥2.0 | 1–2 | <1 |
| PHT (ms) | <130 | 130–200 | >200 |
Some normal values in Tables A5 and A6 cross these cut-offs (mean gradient 8.3 ± 1.71 mmHg for the 25-mm Hancock II), and the guideline notes slightly higher cut-offs in some bioprostheses. Judge against that valve's own normal values and the baseline study.
| PPM, Adult-Derived · ASE 2024, Table 7 Valve structure and motion are normal and the measured EOA is within 1 SD of the reference EOA. Index the continuity EOA (not PHT-derived) to BSA. The link between indexed EOA and gradient is weaker than in the aortic position. | |||
|---|---|---|---|
| Indexed EOA (cm²/m²) | Normal | Moderate | Severe |
| BMI <30 kg/m² | >1.2 | 1.2–0.91 | ≤0.90 |
| BMI ≥30 kg/m² | >1.0 | 1.0–0.76 | ≤0.75 |

| When to suspect stenosis, Adult-Derived · ASE 2024, Table 19 and text Also in the text
The guideline presents its approach as one developed in adults and says it generally works in congenital heart disease, with limits: serial stenoses, shunts, eccentric jets, noncircular outflow tracts and body size (Section VI, Table 23). | ||
|---|---|---|
| Suggests stenosis | Bioprosthetic | Mechanical |
| Peak E velocity (m/s) | ≥2.1 | ≥1.9 |
| Mean gradient (mmHg) | ≥9 | ≥6 |
| PHT (ms) | ≥200 | ≥130 |
| EOA (cm²) | <1.5 | <2.0 |
| DVI (VTIPrTV / VTILVOT) | ≥3.3 | ≥2.1 |
Some normal values in Table A9 cross these cut-offs (EOA 1.34 ± 0.22 cm² for the 27-mm Carpentier Edwards Duraflex), and the guideline suggests prosthesis type-specific cut-offs. Judge against that valve's own normal values and the baseline study.
